We partner with family members and other stakeholders to create a unique care plan to address the specific needs of each person. We are dedicated to every client living their healthiest, happiest, and most independent life.
Geriatric Care Management is available to people experiencing cognitive decline, dementia, or medical complexity. We also offer case management for special needs children and adults experiencing other types of disability.
There are a significant number of older adults who gradually lose the capacity to manage finances, make decisions or act in a timely manner to safeguard their own health and welfare. This naturally causes concern for the seniors and their relatives who are not in a position to assume that responsibility. In Illinois, the court is empowered to appoint guardians for adults who are unable to manage their affairs for their own benefit. Our team has firsthand knowledge of how to navigate these situations, preventing neglect, abuse, and exploitation.
Recognizing that declining health and disability can sometimes impede one's ability to make their own medical and residential decisions, we can step in to assist as Guardian of Person. We know this can be difficult and emotional, and make every effort to find the best way forward.
Safeguarding assets and preventing exploitation is a key responsibility of estate management. We can manage household and financial affairs, assist with real estate and personal property, and make sure assets are insured and protected. We maintain integrity and transparency in every interaction.

Serving as your Power of Attorney for Healthcare or Property, we will ensure you and your assets are safe if there comes a time when you cannot manage your own personal or financial decisions. We will get to know your priorities and values and help implement a plan for the future. Once appointed as your Power of Attorney, we serve as a resource throughout the years.
A Durable Power of Attorney is a legal document executed by an individual (known as the principal), that authorizes another individual (known as the agent), to act on the principal’s behalf during his/her lifetime. Although a Durable Power of Attorney is valid at the time it is executed an agent usually does not begin acting on the principal’s behalf until such time as the principal is unable to act on his/her own.
